Puget Sound Hospice is seeking an experienced and caring full-time Palliative Care Nurse Practitioner (NP) in the greater Thurston County, Washington area to help us build our palliative care program! Benefits $50-$75 an hour, DOE Palliative Care Program structure (templates, SME assistance, etc.
) and training provided but this is a new program for this agency.

A palliative care nurse practitioner plays a crucial role in providing comprehensive care to patients with serious illnesses, focusing on pain and symptom management, as well as addressing the emotional, social, and spiritual needs of both patients and their families.

Essential Duties & Responsibilities Patient Assessment:
Conduct thorough assessments of patients with serious illnesses, including symptom management, psychosocial needs, and advance care planning.
Treatment Planning:
Develop and implement individualized care plans in collaboration with the interdisciplinary team, focusing on pain and symptom management, emotional support, and quality of life.
Medication Management:
Prescribe and manage medications to address symptoms, pain, and other palliative care needs, while considering potential interactions and side effects.
Communication and Counseling:
Provide clear, compassionate communication to patients and their families, offering support, education, and counseling on treatment options, prognosis, and end-of-life care.
Collaboration:
Work closely with physicians, nurses, social workers, chaplains, and other healthcare professionals to ensure coordinated, holistic care for patients and their families.
Support for Families:
Offer guidance and support to patients' families, addressing their emotional, spiritual, and practical needs throughout the palliative care process.
Education and Advocacy:
Educate healthcare staff, patients, and families about palliative care principles and advocate for the integration of palliative care in various healthcare settings.
Documentation:
Maintain accurate and thorough documentation of patient assessments, care plans, interventions, and patient/family interactions in accordance with regulatory and organizational standards.
Requirements and Qualifications Advanced Practice Nursing Licensure:
Hold a current, unrestricted nurse practitioner license.
Possess a Master's or Doctoral degree in nursing with a specialization in palliative care or a related field.
Preferred but not required:
certification as a nurse practitioner in palliative care, such as the Advanced Certified Hospice and Palliative Nurse (ACHPN) or Advanced Certified Hospice and Palliative Nurse Practitioner (ACHPNP) credential.
Have a strong clinical background in palliative care, hospice care, oncology, or chronic illness, with experience in pain and symptom management, end-of-life care, and communication with seriously ill patients and their families.
